provides nursing and hygiene care at home for elderly people or individuals with disabilities, delivered by qualified healthcare professionals.<br \/>It is intended for people who wish to remain in their own homes while benefiting from support tailored to their health condition and level of independence.<\/p><p>The hncs\u2019s aim is to prevent loss of autonomy, support staying at home, and improve the quality of life of its beneficiaries.<\/p><hr \/><h2>Our missions<\/h2><p>The SSIAD intervenes to:<\/p><ul><li><p>Provide hygiene and comfort care<br \/><em>(washing, mobility support, dressing, prevention of pressure sores, etc.)<\/em><\/p><\/li><li><p>Deliver or coordinate prescribed nursing care<\/p><\/li><li><p>Support home care in order to avoid hospitalization or facilitate discharge and return home<\/p><\/li><li><p>Contribute to prevention and health education<\/p><\/li><li><p>Support loss of autonomy with kindness and respect for the person<\/p><\/li><li><p>Coordinate interventions with physicians, nurses, physiotherapists and caregivers<\/p><\/li><\/ul><p>The service operates through a comprehensive approach combining care, prevention, support and coordination.<\/p><hr \/><h2>Beneficiaries<\/h2><p>The HNCS is intended for:<\/p><ul><li><p>People aged 60 and over who are ill or experiencing loss of independence<\/p><\/li><li><p>Adults with disabilities<\/p><\/li><li><p>Individuals requiring temporary support after hospitalization<br \/><em>(follow-up care, reduced mobility, temporary frailty)<\/em><\/p><\/li><\/ul><p>Admission is made on medical prescription, following an assessment of needs at home.<\/p><hr \/><h2>A professional and committed team<\/h2><p>Care is provided by a multidisciplinary team including:<\/p><ul><li><p>Coordinating nurses<\/p><\/li><li><p>Nursing assistants<\/p><\/li><li><p>Possible involvement of partner professionals: independent nurses, physiotherapists, occupational therapists, etc.<\/p><\/li><\/ul><p>The team operates <strong>7 days a week<\/strong> according to beneficiaries\u2019 needs, respecting their lifestyle and privacy.<\/p><hr \/><h2>Our approach:
