Hospital Centre has <strong>7 beds<\/strong> and welcomes patients requiring medical care, physiotherapy, and rehabilitation following an acute episode or a hospital stay.<\/p><p>The purpose of this unit is to support patients in regaining independence and returning to their optimal level of functioning, through a personalized care plan adapted to each patient\u2019s abilities and life project.<\/p><hr \/><h2>Our objectives<\/h2><p>The SMR unit provides:<\/p><ul><li><p>Continuation of medical care following an acute phase<\/p><\/li><li><p>Functional rehabilitation to regain strength, mobility and independence<\/p><\/li><li><p>Support for patients experiencing temporary or long-term loss of autonomy<\/p><\/li><li><p>Prevention of complications related to immobility or frailty<\/p><\/li><li><p>Preparation for discharge home or referral to an appropriate facility<\/p><\/li><\/ul><p>Our approach is comprehensive, combining medical, functional, psychological and social dimensions.<\/p><hr \/><h2>Conditions and situations treated<\/h2><p>The unit provides care in particular for:<\/p><ul><li><p>Post-hospitalization recovery following acute medical conditions<\/p><\/li><li><p>Rehabilitation after falls, sedentary periods, or loss of independence<\/p><\/li><li><p>Post-surgical follow-up<\/p><\/li><li><p>Chronic diseases and multiple conditions in elderly patients<\/p><\/li><li><p>Progressive recovery of functional abilities following frailty<\/p><\/li><\/ul><p>Each patient benefits from an individualized care plan and a rehabilitation program
