Medical and Rehabilitation Care

General presentation

The Medical and Rehabilitation Care (SMR) unit at the Irénée de Bruyn Hospital Centre has 7 beds and welcomes patients requiring medical care, physiotherapy, and rehabilitation following an acute episode or a hospital stay.

The purpose of this unit is to support patients in regaining independence and returning to their optimal level of functioning, through a personalized care plan adapted to each patient’s abilities and life project.


Our objectives

The SMR unit provides:

  • Continuation of medical care following an acute phase

  • Functional rehabilitation to regain strength, mobility and independence

  • Support for patients experiencing temporary or long-term loss of autonomy

  • Prevention of complications related to immobility or frailty

  • Preparation for discharge home or referral to an appropriate facility

Our approach is comprehensive, combining medical, functional, psychological and social dimensions.


Conditions and situations treated

The unit provides care in particular for:

  • Post-hospitalization recovery following acute medical conditions

  • Rehabilitation after falls, sedentary periods, or loss of independence

  • Post-surgical follow-up

  • Chronic diseases and multiple conditions in elderly patients

  • Progressive recovery of functional abilities following frailty

Each patient benefits from an individualized care plan and a rehabilitation program adjusted to their health status.


A dedicated multidisciplinary team

Care is delivered by an expert and committed team:

  • A physician responsible for medical follow-up

  • Nurses

  • Nursing assistants

  • Physiotherapist

  • Functional rehabilitation team

  • Psychologist when needed

  • Social worker to organize discharge home or referral

Each professional works in a coordinated manner to ensure comprehensive and consistent follow-up.

  • Preserve or restore independence

  • Respect each patient’s pace and capabilities

  • Promote mobility and autonomy

Admission

Admissions to the SMR unit take place:

  • Following a hospital stay (medicine, surgery, etc.)

  • On medical indication

  • In coordination with local care teams and primary care physicians

An admission assessment is carried out to define rehabilitation objectives and the discharge plan from the very beginning of the stay.
